New Patient Registration

You can register online here, or in person at the surgery by:

  • Completing a GMS1 form (available from reception or can be downloaded here)
  • Patient Health Check form – available from reception
  • Proof of address
  • Proof of Identification

The registration health check will be carried out by trained staff member Monday-Friday between 4:30pm-5:30pm
